According to the Internet, one supposedly receives birthday items from residents when gifting them birthday cakes. All I'm receiving is clothing items and random furniture.

How does one obtain the other birthday items without waiting a whole year?

It's random. If there's something specific you want, I suggest going online and trading. I got a birthday sign while my mother got a K.K birthday and the birthday table.

Its a pain to get the one Birthday gift you want. Of course you have to build enough friendship with 3 villagers just so they remember you really well and they surprise you on your birthday. The only issue is that you can only this once a year.

Perhaps you know this, but maybe some others will be informed.

On your in-game birthday an islander will greet you at your door when you open the game and take you to someone's house. There will be a pinata hanging from the ceiling that you hit with the stick they give you-by punching the A button as fast as you can after they say GO. A number of cupcakes fall to the floor depending on how fast you hit the pinata...I got eight last week. Take them and gift them to your islanders. I don't remember what I got in 2020, but last year I got most of the birthday items in return.

I might tt back and see if I can re-do my birthday, unless someone replies with whether that's possible?
