I've seen a couple of threads about this, so here is a comphrehensive guide on IV's and the common questions about it, complete for how to incorporate this into your Masuda Method. Definitions are in bold, common questions are in green, and answers are below the questions.

BEFORE YOU EVEN THINK ABOUT IV BREEDING, YOU MUST HAVE ACCESS TO KILOLUDE CITY, HAVE A FLECHINDER WITH FLAME BODY ABILITY AND FLY, AND PATIENCE. THIS MAY TAKE 50 EGGS OR 10. LUCK.

Optional:
  1. Hatching O-Power 3

What does IV stand for?
IV's stands for Individual Values. IV's are non-changeable and are decide on a birth of a pokemon(if you hatched it) or if you caught it in the wild. However, while you can't change them after you've caught/bred the pokemon, you can influence them to be bred onto that pokemon. This will make more sense later. IV's go through 0-31.
IV's can be checked in the Pokemon Center in Kilolude City. The guy who looks like an Ace Trainer judges your IV's. When he says "These stats....they can't be beat!" that means you have got perfect IV's in the stats he listed. Anything less and you've got work to do.

For example, a Gardevoir at level 100 with 31 IV's in HP: 301->270. While it's below level 100, the diffference isn't all that much. A 6IV Ralts only has a one-two number difference from a normal ralts at level 1.

How do I get them?
For those who don't know, you breed by putting two pokemon in the daycare which follow all of these requirements:
  1. They must be opposite genders. For example, you cannot breed a male Axew with a male Bagon. There, however, is an exception, in Ditto.
  2. They must be in the same egg group which are located here. NOTE: UNDISCOVERED POKEMON CAN NOT BREED.

Ditto can breed with any pokemon not a legendary(unless you count Phione/Manaphy, THOSE ARE THE ONLY TWO). Ditto, y' know, that blob of goo you find in pokemon village and friend safari?

Ditto is what you'll most likely be using.

First, you need to find someone with a friend safari with Dittos, found only in normal type safaris with THREE slots. Ditto can only be in the third slot.

Second, catch the Ditto's. Nicknames don't matter, right NOW. Ditto's are amazingly annoying to catch, so bring lots of Great/Dusk(night time)/Ultra balls. You need around 20~ if you have an average run with IV's.

Third, check their IV's with the guy in Kilolude City mention earlier. It's best to do with 3~ at a time, because you have to remember the IV's for each one. Or you can write them down. Use your amazing Flechinder/Talonflame with Flame Body and Fly to fly to Camphier Town Pokemon Center. Talk around 'till you find the man who renames your pokemon for you. Simply skip his text by repeadeatly pressing "A." Rename the Dittos in accordance with their stats. For example, one with perfect IV's in HP and Speed you could nickname HPSpd. This will help AMAZINGLY.

If you don't want to rename all Dittos, simply take them to the PC Box and mark them accordingly, 1st one is HP, 2nd is Attack, etc. This saves time.

Fourth, grab your flechinder/talonflame and empty your inventory except for it. Grab the pokemon you wish to make IV'ed. Make sure it's the right nature.

Fifth, give the pokemon you wish to make IV's out of and give it the item known as the "Everstone". This guarantees that the pokemon passes down the nature.

Sixth, double check and make sure that you have everything on the Supply List:

  1. All the Power Items. You get them from the Battle Maison in Kilolude City.
  2. All of your Dittos nicknamed and ready to go.
  3. The item known as "Destiny Knot." You don't need this now, but you will. Trust me.
  4. Patience. I CANNOT STRESS THIS ENOUGH.

Now you are ready.

Take the pokemon with the everstone and place it into the Pokemon Daycare on Route 7. Give the Ditto with the appropriate stats the corresponding Power Item. For example, if you had a Ditto with HP and Speed you would give it the Power Item that improves that stat after battle. After you've given the Ditto the Power Item, place it in the Daycare. Get your trusty bike(it really is trusty) and ride up and down on Route 7 until the man in front of the daycare turns around. Talk to him by spamming the "A" button. Once you've got your trusty egg, go to Centrico Plaza in Lumoise City and ride around. MAKE SURE YOU HAVE YOUR FLECHINDER IN THE TOP SLOT.

****NOTE****
You don't have to fly to Lumoise City. You can ride up and down on Route 7. However, I only stay on Route 7 to get the eggs then hatch them in Lumoise City. It all goes down to you. However, I will say as I do it, and when you see "fly to centrico plaza" you may stay on Route 7 and hatch, IF YOU WANT TO.

Once it hatches, you have a baby with one perfect IV. For this example, the baby would have the HP stat from the ditto.

Go to the daycare, go to the man, spam the "B" button, and go inside and fetch both pokemon. Take the Everstone from the non-ditto. You no longer need this pokemon. Take the Power Item from the Ditto. You should keep this Ditto for later purposes.

Take a different Ditto with two IV's NOT in the HP stat. Maybe this Ditto has IV's in the Attack and Speed stat. Give it the Power Item that passes on Attack. Give the brand new baby the everstone. Put both in the day care center.

Switch to the Super Training screen(the punching bag screen) and it shows you how many eggs you have. Right now, it's just your Fletchinder. Go have a party on Route 7 with your bike until you get five eggs. Hatch them all. Take them to Centrico Plaza and hatch them. This will be your normal routine for right now.

Take them to the IV checker until he says two stats "can't be beat!". Replace the Ditto, again, with a ditto in two different perfect IV's. Get the Power Item from the old Ditto and get the Everstone from the baby. Give the Destiny Knot, an item that makes sure that 5IV's are passed down, to the Ditto. Your baby will ALWAYS hold the everstone. No contest.

Repeat your party on Route 7 with getting five eggs again.

Hatch them again. Check them again. Keep hatching until you get a pokemon in which the IV dude says three stats cant be beat. Basically, you want to keep on doing this, replacing, hatching, and everything until you get a baby with four perfect IV's. Repeat until you get a 5IV pokemon. Congratulations! All your hard work has finally paid off. 5IV pokemon have around a ~30 stat difference from normal pokemon at level 100, giving you the edge in battling. If you want 6IV, breed two 5IV parents(no dittos, yay) and give one the everstone and the other destiny knot. You have a 1/200 chance of hatching a 6IV pokemon.

It took me 100+ eggs for my 6IV Ralts. You need patience.

I want a 5IV shiny!
You're going to need a pokemon from another region, and it needs to be 5/6IV. Not necessarily, but it helps. You could also use a 6IV pokemon with the right nature and give it the everstone, and give the foreigner the destiny knot. You have a smaller chance than with a 5IV foreign of hatching with 5IV's, but it works. Breed them together if you want a shiny. THIS MAY TAKE AN EXTREMELY LONG TIME OR EXTREMELY SHORT. IT DEPENDS ON YOUR LUCK.

I don't have patience!
Buy one. Hope you have the TBT. I IV breed for a price. Interested? Check here.

Hope this helped! If it did, a post below would be warmly received :D.
Any feedback is also appreciated!

1 thing. When doing the IVs, you can use the markers on the bottom at the PC or summary of a pokemon to mark IVs. The 1st one is HP, 2nd Attack, and it goes in order of the stats on the line in summary.

Good tip. I'll add that.

Thank you so much for this! IV's were always something of confusion for me but this guide completely clears up everything!

Just adding, you don't need access to Kiloude City. You can import it from an earlier generation or trade with someone as well.

This is a guide for breeding pokemon in this generation, so I don't see how you wouldn't need access.

Oh yeah, forgot that the IV checker is there. XD He's kinda important.

I'd recommend just staying at Route 7 to save the tediousness of flying back and forth to Lumiose City.

It all depends, really, on what you prefer. Lumoise City is if you want to go AFK or something and have an egg ready or you don't want to go back and forth. Route 7 is if you plan to stay in one area.

Question

I hatched an eevee and it inherited the perfect attack IV from the ditto. I then swapped the ditto for a different one. This one has perfect defense. I bred it with the recently hatched eevee with perfect attack. But the new baby only had perfect defense and not attack.

Is there a way to get both IV passed down or do I just need to keep trying until attack and defense becomes perfect?

I already pretty much know what I'm doing when it comes to IV breeding, but how do people get 6 IV pokemon? I went a looonnnngg time breeding 5 IV pokemon just waiting for one with the right nature, and never got a 6 IV one.

Or go to Anistar City because it can say hatched in Anstar City, the best place in Kalos.

It's just random. Keep trying. It's still way more common than shinies.

Keep trying. It should work eventually. What eeveelution are you trying to get?
