I have:
123 Trees
89 Flowers
No trash on the ground or weeds
and the max amount of public works projects I can have

But i still don't have a perfect town, I talked to Isabelle and she said they want more public work projects but when i try to make one it says i am at my max

I GIVE UP xD

That's odd. Do you have any normal items on the ground? If you have over ten of anything at all, you can't get a perfect town.

Maybe you have too many illuminated public works (they light up)?

I've heard that since they're bad for the environment, they bring down your rating. There's some other things that bring down your rating, can't remember

Oh okay I have one illuminated project, I'll destroy it and see if that fixes it.

I've heard you can destroy projects by making a new character and making your house right on top of it, but I'm not sure if it's true. Might save you a lot of days trying to destroy stuff, though lol

You can destroy them by talking to Isabelle and i don't care about bells I have 75 million just sitting in my bank account xD
