Backwater Clinic?

Hi! So, I just got HHD (Obsessed), but I've run into a bit of an issue:
What in the world is a "Backwater Clinic"?
This was Gaston's request, And I can't figure out what that means! Please help! Thank youuuu

A "backwater" is a place in the middle of nowhere. Like, little towns that are almost not on the map. So basically he's looking for a little, local clinic. You could make it sketchy if you wanted, or like a small-town doctor's office, or even voodoo-like.

"Backwater" generally means a place that is isolated. So, if you made the kind of clinic you imagine would exist by itself in a forest or countryside, I think that would fit what Gaston is looking for.
