is there much of a difference from photoshop cs4 extended?

Not much really. In CS4 the layout has changed and there's a few more filters and different ways of doing things. Also imageready is built straight into photoshop where as in CS2 they're seperate applications.

I personally prefer the old CS2 style layout so that's the reason I use it.

That would be impossible, I don't ever remember how I make any of my signatures seeing as I change bits here and there all the time. There is no set method to my graphic design, I just do whatever looks good and use a few tricks I've learnt along the way.

Next time maybe make it less simplistic: add some filters, blur, C4D, all that good stuff.
