Purchased Username Change. How do I use it?

Hey everybody. After a long hiatus I've decided to finally get back in to Animal Crossing. r/AnimalCrossing just isn't as friendly as the good ol' Bell Tree Forums. My username reflects an old username that I've since abandoned, and purchased a Username Change. How can I use it?

go to the contact the staff section of bell tree hq. make a post but select the category "username change"
