I saw somewhere that somebody could expand the hospital at 40 clients. I'm not sure if that's true or not.
I just tried it out for myself and nothing

You can add a wing by talking to the receptionist and donating 120k poki. You can expand the restaurant after arranging three roommate move-ins

Hospital needs a Poki donation to the receptionist.

Cafe and restaurant upgrade after you make multiple roommates. Don't know the exact numbers, but I think that Restaurant is at 3 Roommates.
