the costs of killing the doc is very high as opposed to just a VT v a goon

lucky wants to get shot and claiming a PR is a good way of making that happen

if you believe one of us to be doctor, then one of us will either successfully save our target, or die and flip doc

if mafia doesnt know if there actually two doctors and they are presented with the possibility that the two are protecting each other they will give up attacking any of them hence protecting the real doctor

Him being lucky is a possibility but it's a slim possibility and if he is the lucky, it will be somehow relevant for him to claim it now that you mentioned the possibility.
