In the Japanese, you're not actually choosing the suffix, which is 島. You're choosing the pronunciation.
The reason there are 3 versions is because of 'on', 'kun', and 'rendaku'.
'On': means sound. Since kanji are characters taken from Chinese and imported into Japanese, this reading gives something closer to the original Chinese pronunciation of the character
'Kun': means meaning. This was about trying to read the kanji character using the pronunciation of the Japanese word for it that already existed, that is, using the meaning of the kanji to govern its pronunciation.
'Rendaku': sequential voicing, which governs how to pronounce the consonant of the 2nd part of a compound word (formed by putting two whole words together). So "k" becomes "d", "t" becoms "d", "sh" becomes "j", "h" becomes "b". The name of the brand "tokidoki" is governed by this. It's actually "toki" + "toki" = tokidoki. So the word "sometimes" is formed from saying "time time", but changing the first consonant of the second word so you understand it's a compound word. Like if saying "timedime" meant "sometimes" in English.
So here:
'On' reading: とう (tou)
'Kun' reading: しま (shima)
'Rendaku': じま (jima)
As you can see, there's literally no equivalent for this in English. You don't pick the pronunciation of "island".
